When filling the form please use your name as it appears on your score report.
Create a zoom account, this will be needed to hold online meetings.

After you apply

We will contact you to get a copy of your USMLE score report with your name on it (It will be available for students to see), and to get the picture you wish to use as a profile picture. The picture does not have to be formal, it is up to you.
Your submission will be reviewed and verified.
Your profile will become under construction.
You will be notified once it is up and running.
